- Take a survey related to a in store pharmacy purchase
The survey will take less than 5 minutes to complete and all results are compiled by the InMoment survey company. To start the patient experience survey the customers will need the survey inviation code that can be found on the customer receipt. Customers who do not have this code will not be allowed to participate in the survey or sweepstakes.

- Customers will have 72 hours to take the Your Pharmacy Cares survey from the time of purchase
- Customers can also take the survey via phone by calling 858-879-CARE
- Lucky winners will take home a brand spanking new Apple Ipad
- The odds of winning the Apple Ipad will depend on how many people enter the promotional survey contest
- The Your Pharmacy Cares survey code will be 7 digits long and found at the bottom of the receipt
